The present study aimed to determine the effectiveness of cognitive-behavioral therapy (CBT) in reducing alexithymia and psychological distress among patients with chronic pain disorder. This research employed a quasi-experimental design with a pretest–posttest and control group with a two-month follow-up. The statistical population included individuals diagnosed with chronic pain disorder who attended the Imam Ali (AS) Pain Clinic at Arefian Hospital and the Shamim Psychological Center in Urmia during autumn and winter 2024. A total of 30 participants were selected through purposive sampling and randomly assigned to experimental and control groups (15 in each). The experimental group received eight 90-minute sessions of CBT. Data were collected using the Toronto Alexithymia Scale (TAS-20; Bagby et al., 1994) and the Depression Anxiety Stress Scales (DASS-21; Lovibond & Lovibond, 1995). The findings showed a significant difference between pre-test and post-test scores of alexithymia and the components of psychological distress in the experimental and control groups at the 0.01 level. Additionally, there was no significant difference between post-test and the two-month follow-up mean scores in the experimental group at the 0.01 level, indicating the results were maintained during follow-up. Based on these results, cognitive-behavioral therapy is an effective intervention for reducing emotional and psychological problems in patients with chronic pain disorder.
