The present study aimed to compare the effectiveness of Emotion Efficacy Group Therapy and Mindfulness‑Based Cognitive Group Therapy on pain management and psychological distress in women with chronic low back pain. The research employed a quasi‑experimental design with a pretest–posttest control group design and a two‑month follow‑up period. The statistical population consisted of women with chronic low back pain who referred to hospitals and specialized clinics in Tehran in 2025. From this population, 60 individuals were selected through convenience sampling and randomly assigned to two experimental groups and one control group. Data were collected using the Short‑Form McGill Pain Questionnaire (SF‑MPQ; Melzack, 1975) and the Kessler Psychological Distress Scale (K‑10; Kessler et al., 2003). Data analysis was performed using repeated‑measures ANOVA and Bonferroni post‑hoc tests. The results revealed a significant difference between pre-test and post-test scores for both pain management and psychological distress in the experimental and control groups  (P<0.01). Regarding the follow-up phase, a significant difference was observed between the post-test and follow-up scores in the emotion-efficacy-based therapy group (P<0.05). Conversely, no significant difference was found between the post-test and follow-up scores in the mindfulness-based cognitive therapy group (P>0.05); the therapeutic gains remained stable during the follow-up period. Furthermore, no significant differences were observed between the two experimental groups (P<0.05). Consequently, both treatments were effective interventions for improving pain management and reducing psychological distress in women with chronic low back pain.
