The present study aimed to predict the neurotic defense mechanism based on death obsession and emotional control in patients with heart failure. The research method was descriptive-correlational. The present study is an applied research in terms of purpose and a correlational study in terms of method. The research population consisted of all patients with heart failure on Kish Island in 2022-2023, from which 310 people were purposefully selected as a sample. The instruments of this study included the Defense Styles Questionnaire of Bond et al. (1993, DSQ-40), the Death Obsession of Abdul Khaliq (1998, DOQ), and the Emotional Control of Williams et al. (1997, ACQ). The analysis method of this study was simultaneous regression analysis. The findings showed that death obsession and emotional control can predict neurotic defense mechanisms, and about 18.1% of the variance of neurotic defense mechanisms in patients with heart failure is predicted by death obsession and emotional control (P<0.05). From the above findings, it can be concluded that death obsession and emotional control play a fundamental role in the occurrence of neurotic defense mechanisms in patients with heart failure.
