This study aimed to compare the effectiveness of acceptance and commitment therapy (ACT) and mindfulness-based cognitive therapy (MBCT) on psychological flexibility in individuals with major thalassemia. The research method was a quasi-experimental design with a pretest-posttest design and a control group with a three-month follow-up period. The statistical population of the study included patients diagnosed with major thalassemia who had been referred to Shahid Rajaee Hospital in Tonekabon in 2024 for follow-up treatment. The sample size consisted of 45 people who were selected by convenience sampling and randomly assigned to three groups of 15. The research instruments included the Acceptance and Action Questionnaire-Second Edition by Bond et al. (2007). The ACT and MBCT group received 8 sessions of intervention 2. Data were analyzed using repeated measures analysis of variance. The findings showed that in the two intervention groups, ACT and MBCT, the scores from pre-test to post-test and follow-up of psychological flexibility significantly increased compared to the control group (P<0.05); the findings also showed that there was a significant difference between the two intervention groups at the follow-up stage (P<0.05). It can be concluded that ACT was more effective than MBCT in increasing psychological flexibility among patients with major thalassemia.
