The present study aimed to determine the effectiveness of trauma-focused mentalization-based group treatment on ambivalence over emotional expression and self-critical rumination in women with adverse childhood experiences. The research method was a quasi-experimental design with a pretest-posttest design and a control group with a two-month follow-up. The statistical population consisted of women with ACEs and high levels of mental pain in the city of Zarrinshahr in 2025. Through purposive sampling, 40 women were selected and randomly assigned to two experimental and control groups of 20. The measurement tools included the Adverse Childhood Experiences Questionnaire (ACEs-10; Felitti et al., 1998), Mental Pain Scale (MPS; Orbach et al., 2003), Ambivalence over Emotional Expression Questionnaire (AEQ; King & Emmons, 1990), and Self-Critical Rumination Scale (SCRS; Smart et al., 2016). Data were analyzed using repeated measures ANOVA. The findings showed that there was a significant difference at the 0.001 level between the pre-test and post-test in ambivalence over emotional expression and self-critical rumination in the experimental and control groups, and there was also no significant difference at the 0.001 level between the post-test and follow-up mean in the experimental group, and the results were maintained during the follow-up. Based on this, it can be concluded that the use of trauma-focused mentalization-based group treatment is effective in reducing ambivalence over emotional expression and self-critical rumination in women with ACEs and high levels of mental pain.
