The present study aimed to examine the relationship between anxiety sensitivity and illness anxiety, with the mediating roles of negative metacognitive beliefs and negative meta-emotion, in older adults. This study was descriptive–correlational in nature and was conducted using structural equation modeling. The statistical population of the study consisted of adults aged 60 years and older in Tabriz city in 2025, from whom 236 individuals (112 men and 124 women) were selected through convenience sampling and completed the questionnaires. The research instruments included the Health Anxiety Inventory (HAI; Salkovskis & Warwick, 2002), the Anxiety Sensitivity Index (ASI; Taylor & Cox, 1998), the Metacognitions Questionnaire (MCQ; Wells, 1996), and the Meta-Emotion Scale (MES; Mitmansgruber et al., 2009). The data were analyzed using structural equation modeling. The results indicated that the proposed model showed an adequate fit. The findings demonstrated that anxiety sensitivity was able to directly predict illness anxiety in older adults (p < 0.001). Moreover, anxiety sensitivity was able to indirectly predict illness anxiety through negative metacognitive beliefs and negative meta-emotion (p < 0.001). These results indicate that negative metacognitive beliefs and negative meta-emotion play an important role in explaining the relationship between anxiety sensitivity and illness anxiety in older adults, and that attention to these constructs in interaction with psychological factors is necessary.
