This study aimed to determine the effectiveness of spiritual-religious psychotherapy with an Islamic approach on existential anxiety and attitudes towards death in the elderly. The research method was a quasi-experimental design with a pre-test-post-test design with a control group. The statistical population included all male elderly members of the nursing home in Ardabil in the second half of 2023. The number of study subjects was 36 people who were selected by purposive sampling and randomly assigned to the experimental group (n=18) and the control group (n=18). The experimental group received 10 sessions of 75-minute spiritual-religious psychotherapy intervention with a revised Islamic approach, once a week. The death attitude profile - revised (DAP-R, Wong et al., 1994) and existential anxiety questionnaire (EAQ, Weems et al., 2004) were used to collect data. Data analysis was performed using multivariate analysis of covariance. Based on the findings, it was shown that by controlling for the pre-test effect, there was a significant difference at the 0.01 level between the mean post-test of existential anxiety and attitude towards death of the elderly in the two groups. Considering these results, it can be stated that providing spiritual-religious psychotherapy with an Islamic approach had a significant effect on reducing existential anxiety and improving the attitude towards death of the elderly.
