The present study aimed to investigate the relationship between negative emotional states and mild cognitive impairment in elderly people. This systematic review and longitudinal meta-analysis examined the relationship between negative emotional states and mild cognitive impairment in older adults. This study was a systematic review and longitudinal meta-analysis that searched for related keywords using MESH through PubMed, Web of Science, and Scopus databases from October to November 2024, based on the PRISMA guidelines. Finally, eleven studies remained based on the inclusion and exclusion criteria. Effect size and variance estimates were calculated using the "metafor" and "robumeta" packages in RStudio (2024.12.1+563). The results of the meta-analysis indicated a positive effect of depression on mild cognitive impairment in the elderly (the mean effect estimate of depression was 0.185 (P=0.009)). The results of meta-regression on year of publication and sample size showed that these two did not affect the result (p>0.05). Sensitivity analysis was also performed based on different correlations. The results showed that there was no change with different values of correlation. Considering the direct effect of depressive symptoms on mild cognitive impairment in the elderly, depression is considered as a real risk factor for mild cognitive impairment. These results should be considered by experts and clinicians in order to control and manage cognitive impairment in the elderly.
