The present study aimed to investigate the mediating role of alexithymia and mindfulness in the relationship between childhood trauma and psychological distress in individuals with chronic pain. This research employed a descriptive-correlational design using structural equation modeling. The statistical population consisted of individuals with chronic pain in Neyshabur in 2024 who had referred to medical centers. A total of 255 participants were selected using convenience sampling and responded to the Childhood Trauma Questionnaire (CTQ; Bernstein et al., 2003), the Kessler Psychological Distress Scale (K-10; Kessler et al., 2002), the Toronto Alexithymia Scale (TAS; Bagby et al., 1994), and the Five Facet Mindfulness Questionnaire (FFMQ; Baer et al., 2012). Data were analyzed using path analysis. The results indicated a good model fit. Childhood trauma had a significant and direct effect on both alexithymia and mindfulness. Furthermore, alexithymia and mindfulness had significant direct effects on psychological distress (p<.05). Additionally, the findings revealed that alexithymia and mindfulness mediated the relationship between childhood trauma and psychological distress (p<.05). It can be concluded that childhood trauma affects psychological distress indirectly through the mediating roles of alexithymia and mindfulness.
